If you are under the age of 18 when you complete your New Start contract, you must complete a Parent Permission Form in order to guarantee your spot in the program.
Please download the Parent Permission Form. This form must be completed by you (student) and your parent or guardian before May 15, 2023!
You may upload this document on your UA Future account if you have not yet completed the contract. If you have already submitted your contract you will need to send the completed form to our office. Please email to newstart@arizona.edu or fax the form to 520-621-3799.
Once we receive the Parent Permission Form we will send a confirmation email to your UA email account.
