Please note that this form requires you to log in to your UA Future account and return to your New Start Application. You must complete the cancellation by June 8, 2019 in order to avoid being charged the $150.00 late cancellation fee.
It is your responsibility to reschedule the June 19, 2019 New Student Orientation by contacting New Student Services. To request an alternative date, email orientation@arizona.edu or call (520) 626-0849. A $35 fee will be assessed for rescheduling.
